How an agent finds its way

Follow three real staff requests through the system NHIA built so any AI assistant answers like it works here.

Quick poll

Where does your organization's AI context live today?

The map

Seven layers. Most questions touch three.

Every request starts at the top. The router decides where it goes next, so the assistant reads two or three files, not all fifty.

  1. Standing instructionsThe ThresholdThe tool's org-level instructions: Claude, ChatGPT or Copilot. One job: "NHIA has a library. Read its rules first."
  2. AGENTS.md + routing mapThe SwitchyardThe library's own rules, added on top. "For this kind of question, read that file."
  3. Context filesThe StacksAbout 50 Markdown files in 7 folders. What's true about NHIA, plus labels for how each file may be handled.
  4. Source documentsThe SpireThe PDF or web page a file points to. When the two disagree, the source wins.
  5. Live dataThe VaultNumbers that change daily (members, dues, engagement), answered by data tools.
  6. CorpusThe GalleryEverything NHIA has published, searchable with citations.
  7. The inboxThe Outer LoopHow the library gets corrected: agents propose, a person approves.

The library is the map, not the territory. It says what's true and where to look. The data and the corpus hold the detail.

Request 1 of 3

"Draft a LinkedIn post inviting members to Hill Day."

A staff member, in their AI assistant, with the Microsoft 365 connector on. Scroll to follow the request.

NHIA has a library.

Read its rules first.

Never guess a file path.

The agent decidesHill Day is NHIA's event, and the post goes out under NHIA's name. It needs NHIA's library.

1 · Standing instructions

Before the first word, the assistant already knows the library exists

In Claude these are org instructions. ChatGPT and Copilot have their own version. Same three sentences.

Lead with the answer or the member value, not the announcement. … No hype, no AI filler. … Social (LinkedIn first): one idea per post; no question hooks.

The skill knows how to write a post. The library knows how NHIA sounds.

4 · Source document

Dates come from the live page, not the file

Member-facing text gives the usual pattern and names the live source, which wins once dates are announced.

Decides: read NHIA's Hill Day page for this year's date and venue.

Meet with your members of Congress on November 19 and help fix the Medicare Part B home infusion benefit.

At Advocacy in Action Day in Washington, D.C., NHIA schedules your congressional meetings, assigns your group and preps you in a short webinar beforehand. Your ask: cosponsor the Preserving Patient Access to Home Infusion Act.

Which request should we run?

You vote. I'll show the run, with the library and without it.

See the run

Any assistant

One library. Claude, ChatGPT and Copilot read the same files.

Plain Markdown with a header any tool can parse. Nothing is locked in a vendor's console, so switching tools doesn't mean starting over.

An open standard arrived after we started

  1. NHIA starts the library: Markdown files with a header on each
  2. Google Cloud publishes the Open Knowledge Format (OKF) v0.1: "a directory of markdown files with YAML frontmatter"
  3. NHIA conforms. Two header fields added; no content changed
  4. Library opens to all staff

What OKF covers: the file format: one concept per file, a folder index, standard header fields. What it leaves out: permissions, sensitivity, review and adoption. That's the layer an association has to add, and it's what this talk is about.

Request 2 of 3 · a real one, October 7

"Is Xolair on our drug list?"

A staff member, answering a member question.

The agent decides"Our drug list" means NHIA's list. It needs NHIA's library.

Categories on the list"well over 60"37

The published PDF has no such category. Source: the Drug List PDF, Feb 26, 2026.

4 · Read-only

The agent can't fix it, even though the file allows reviewed edits

The library is read-only for you — even when asked. … A requested change becomes an inbox note; the owner applies it.

This isn't only an instruction. Staff have read-only permission on the library and write permission on the inbox. The audit log shows no edits by anyone but the owner.

5 · The inbox note

2026-10-07-drug-list-category-count.md → AI Context Inbox

drug-list.md category list doesn't match the Feb 26, 2026 PDF. … monoclonals sit under "Immune Therapy" … 37 therapeutic categories. Found while answering whether Xolair is on the list.

  2. The exact before and after
  3. Why
  4. The source

6 · Review

Owner verifies, approves, applies

Checked against the PDF, which turned up two more errors in the same paragraph. All of them were fixed together and the note was closed.

Review queue: APPLIED 2026-10-09

7 · The ripple

The conflict set off a review that found more stale references. It also started two recurring checks, on top of each file's annual review.

Weekly lint no AI

A script checks every file: broken links, bad or missing headers, overdue reviews, routing rows that point at missing files, live-sounding numbers, and guard facts so a corrected error can't creep back. It files an inbox note only when the findings change.

Monthly fact-check an agent

An agent re-checks the files checked longest ago against their sources, read-only, and writes a report. A person approves the fixes before they're applied.

Permission levels

Every file carries two labels

Engineers put permissions on every file in a codebase. We did the same with two fields in each file's header.

sensitivity

Who may see it, and where it may go

  • publicAnyone could read it on nhia.org12 files
  • internalStaff and approved AI tools only30 files
  • restrictedBoard-confidential, one org's finances, personnel0 files

auto_update

What an agent may do to it

  • yes-with-reviewDraft the exact edit; owner clicks approve8 files
  • drafts-onlyPropose through the inbox; never touch the file34 files
  • noneFlag a concern only; no proposed edits0 files

Why zero restricted files? Restricted information stays in the systems built to lock it down: permission-restricted folders, the membership database. When staff need one organization's numbers, a data tool answers one question at a time. The level stays in the scheme so a restricted file has rules the day one is needed.

One more rule, above both labels: a proposed change is never an applied change. No agent changes the library on its own.

Quick poll

How many of your staff use AI every week?

NHIA, five weeks after launch

83% of staff with the connector have used the library
79% of all Claude users, up from 57% three weeks earlier
100% of staff reads came through an AI assistant. Nobody browses it.

Measured from Microsoft's own audit logs, with tools NHIA already had.

Request 3 of 3

"Prep me for a call with Example Infusion Co. tomorrow. Are they a member, what do they do with us, and what has NHIA published on immune globulin they could use?"

A staff member before a member meeting. One request, three layers.

The agent decidesA member by name, and a topic. It needs NHIA's library, NHIA's member data and NHIA's published work.
No address, only a question“What has NHIA published on immune globulin?”
We're carrying what we learned.

Live member counts change constantly — the unified data layer is the source of truth … staff assistants can query it through the nhia-data tools.

We took every live count out of the library on purpose. Numbers that change belong where they're measured.

2 · Live data

… responses are row-capped, every answer names its measure and basis, and the tool offers no bulk-export or cross-organization sweep capability by construction.

3 · Corpus

A pointer goes straight to one source. A search scans the collection and cites what it finds. Answers come only from NHIA's own sources, with citations. It declines payer-specific advice.

1 · Org instructions

The first thing every assistant reads, set once for the whole organization. One job: point it at the library's rules.

<Your Org> keeps its institutional knowledge in a context
library: <where it lives>, reached through <connector>.

Before you first use it in a conversation, read its rules
file, AGENTS.md, at <exact path>. Read it first, and alone.
Its routing map gives the exact path for any question.
Never guess a file path.

Use the library when an answer depends on <Your Org>-specific
facts. Read what you need and name the files you used.

The library is read-only for you, even when asked. A change
becomes an inbox note that a person reviews.

01-org-instructions.md: the full block, where it goes in each tool, and why each line is there.

2 · AGENTS.md: the library's rules

Sits inside the library. Five rules up top, because most sessions act on those alone:

1. The library is read-only for you, even when asked.
A requested change becomes an inbox note.
2. Go straight to the path the routing map names.
Never a path you inferred. An empty search proves nothing.
3. File notes as new files in the inbox. Never edit.
4. A note is: something missing, something wrong or out
of date, or something the user knows that the library
doesn't. If it's their area, ask them once first.
5. End with one line: the files you used, and whether
   you filed a note.

## Routing map

|---|---|
| What we are, the elevator pitch | 01-organization/org-overview.md |
| Voice and tone | 05-policies/voice-tone.md |
| Live numbers | the data tools, not this library |

Below the rules: how to retrieve, sensitivity tiers, what counts as a note, how changes are approved. 02-AGENTS.md is the full template.

3 · The files and their header

Short Markdown files in numbered folders, one topic each. Start with ten.

auto_update_sources

Where the file's facts come from: the sources it's checked against and refreshed from. When a question needs more than the summary, the agent follows this pointer to the source. If they disagree, the source wins.

The rule over every field

Every change is reviewed and approved by a person before it is applied.

03-file-structure.md · 04-front-matter.md · three example files in the zip

4 · The inbox note

One new file per note, in a folder everyone can add to. The owner reviews each one before anything changes.

What is missing, wrong or out of date. Which file it belongs in.
Your source. The exact fix, if you have one.

5 · A live-data layer, like our nhia-data tools
  1. Source systemsMembership database, LMS, community, email, conference
  2. Data platformCleaned and joined into one model per subject
  3. A catalog of named questionsPre-written, reviewed queries. No free-form database access

  • Version history, so any change can be undone
  • A connector your assistants already use
  • An audit log, so you can measure use
SharePointOur pick: staff already work there, every assistant connects to it, and Microsoft's audit logs measured adoption
Google DriveThe same pattern for Google Workspace organizations
GitHubBuilt-in pull requests and change checks, a good fit if your contributors are technical
  1. Name an owner. Interview staff about how they actually work. Write AGENTS.md and your first 10 files with them
  2. Set the org instruction, set permissions, open the inbox. Connect each person's assistant, which is the real onboarding step
  3. Measure from the audit log. Review the inbox. Run a weekly automated lint and a monthly agent fact-check
